What Is Medication Management Therapy?

Medication Therapy Management (MTM) is a specialized service where our clinical pharmacist reviews your prescription drugs, over-the-counter medications, and supplements. This comprehensive review aims to identify and resolve issues such as drug duplications, incorrect dosages, interactions, and other medication-related problems. Our clinical pharmacist may suggest adding or discontinuing certain medications and will communicate these recommendations to your prescribing provider. Additionally, MTM includes regular follow-up sessions to monitor symptom improvement, side effects, and adherence to the medication regimen.

How the Process Works

Our clinical pharmacist starts with an assessment to check your adherence to the prescribed medication regimen. They will also guide the monitoring of your reactions to new or modified medications. The primary goal of MTM is to ensure appropriate medication use, enhance adherence, detect adverse drug events early, and improve collaboration between your prescribing provider and our clinical pharmacist, ultimately leading to better patient outcomes.

Please Note: Medication Therapy Management (MTM) does not include prescribing medications, and our service does not involve writing prescriptions.

    Getting Started with Online Medication Therapy Management

    Step 1
    Schedule a free online consultation with our staff. This consultation helps us determine if our services are appropriate for you. During this session, you will schedule your initial evaluation appointment.
    Step 2
    Complete the new patient paperwork, which will be sent to you before your initial evaluation appointment via our secure, HIPAA-compliant platform. Make sure to submit your paperwork at least two days before your appointment.
    Step 3
    Attend your appointment with our doctor over video using our HIPAA-compliant platform. A link will be emailed to you at least 72 hours before your appointment to access the session.
